Lions Eye Foundation of Northern California-Nevada

Description:

Assists residents of Northern California and the entire state of Nevada requiring eye surgeries. Mission is to preserve and restore the gift of sight by providing free ophthalmic, examinations, operations, and medication to the less fortunate members of our community.

Eligibility:
-Patient must have one year's continuous residency in the communities served by the foundation
-Income must be less than $29,175 for a single person
-Proof of income required
-Patient has no insurance (no county, government, or private coverage, no partial coverage, share-of-cost or deductibles)
-Total assets cannot exceed $75,000

A referral from the patient's doctor is required, along with the referral form, and then sent to the local member club, found on the website, with attention to the "Sight Conservation Chair." All forms are available on their website.
