Description:
Nexus Medi-Cal Navigators: Assist individuals with completing and submitting Medi-Cal applications. Medi-Cal Navigators are experienced case managers who can help simplify the application process for you. Please call them for more information or to schedule an appointment.
Asthma Mitigation Project: Free educational & resource connection referral program for children, adults & seniors diagnosed with uncontrolled asthma. Asthma Education Specialists will help with recognizing signs and symptoms, learn to manage triggers & ways to avoid them, learn how to maintain & properly use your asthma management equipment, practice asthma prevention methods.
