We will help you get control of the ABC's of diabetes:
A - Hemoglobin A1c
B - Blood pressure
C - Cholesterol
Your primary care provider can refer you to the Diabetes Resource Center. Our nurse practitioner will complete an individualized assessment and work together with you to set goals for your success.
Diabetes Support Group - The Diabetes Support Group is for adults living with Type 1 and Type 2 diabetes. Each session includes a topic of interest and enough time for discussion. Light refreshments are also served. The support group is free and no registration is required. Please call 209-536-3728 for further information.
