Long-Term Care Ombudsman Program: 209-588-1597 - A program that advocates for residents in Licensed Skilled Nursing and Residential Care facilities.
Elder Abuse Prevention Program: 209-588-1597 - A program that provides essential elder abuse prevention and mandated reporter trainings to various businesses, agencies, and Long-Term Care facilities.
Mother Lode Wellness Program: 209-396-6953 - A program that provides a variety of services which includes general clinical counseling for children, individuals, couples, seniors, and family.
Supportive Services for Veterans and their Families (SSVF) (VETFAM): 209-396-6903 - A program to serve homeless veteran families and those at-risk of becoming homeless by providing wraparound supportive services such as: rental assistance, utility payments, child care, transportation, medical/mental health referrals, affordable health care, food nutrition assistance and much more to veterans with significant housing barriers. SSVF has Healthcare Navigators who can help clients get connected to the Veteran Affairs and other forms of health care and community referrals.
Catholic Charities Diocese of Stockton's Veterans Program was awarded additional funding through the CARES act. This funding will help provide Veterans with security deposits, utilities deposit, rental arrears, utilities arrears and other necessary funds based on the needs of the clients. Peer Support Social Interaction Group: for Seniors 60 and above. This group will meet weekly to offer individuals a space to interact, learn interpersonal skills, share community resources, and an opportunity to create social connections. Weekly Older Adult Support Group: Starting October 25, 2023 at 10:00am. Join them for a counseling group tailored for individuals ages 60 and above. The group will address various topics including aging and adjustment, bereavement, isolation, stress, and anxiety. Don't miss out on this opportunity to connect with others and receive support in a safe and welcoming environment. Meetings are held twice a month on the 2nd and 4th Wednesday at St. Patricks, 98 West Jackson St, Sonora in the Parish Hall Room 2. Call 209-396-6953 for more information.
Disaster Recovery (Tuolumne and Calaveras Counties): Assists survivors of the Winter 2022/Spring 2023 storms develop a realistic long-term recovery plan. Helps to identify and facilitate access to appropriate community resources that will support the goals/plan established for each survivor. Services include FEMA Appeals, General Case Management, Recovery Assistance, and Connection to Support Systems. Contact for more information.
